INSTRUCTIONS
Those students needing a work permit should download the application below and follow the listed instructions. Please contact Mrs. Haswell in the HS office to process a work permit.
* Student/Applicant Information must be completed by the student and parent/guardian.
* Pledge of Employer Information must be completed by the person who is employing the student.
*The Physician's Certificate must be completed by a licensed doctor OR
a copy of a sports physical given within the last 365 days may be used.
WORK PERMITS CAN NOT BE PROCESSED UNTIL THE FORMS ARE COMPLETE.
